From personal care and household tasks to specialist disability accommodation and community access, My Disability Provider offers the full spectrum of services supported by the NDIS, Victoria, New South Wales and Queensland. What Is The NDIS?

National Disability Insurance Scheme

The National Disability Insurance Scheme (NDIS) is a governmental initiative to provide Australians with disabilities the resources and support they need to live fulfilling, independent lives. It covers a wide range of personal support and services, enabling individuals to participate actively in their communities and pursue their goals.

If you are an Australian citizen or permanent resident between the ages of 7 and 65 and have a permanent and significant disability, you may be eligible for the National Disability Insurance Scheme (NDIS).

The NDIS is a whole-life approach to disability, aiming to provide people with disabilities the resources they need in order to live independent and meaningful lives. The NDIS provides individuals with the opportunity to work towards their goals and lead a life of choice and control.

Assistance With Personal Care & Daily Activities

We provide assistance with personal care and daily activities, such as showering, dressing, and taking medication. Our team of experienced professionals is here to help you maintain your independence and remain safe in your home.

Assistance With Social & Community Participation

We assist our clients with social and community participation, helping them to build relationships with their peers. We provide support with leisure and recreational activities, work placements, and training.

Respite Care (STA) & SIL Accommodation

Our respite care and Specialist Therapeutic Accommodation (STA) services provide short-term relief for both clients and their families. We can also assist with finding suitable Supported Independent Living (SIL) accommodation for individuals requiring more permanent placements.

Nurses For High Needs Participants

For NDIS participants with higher needs, we provide nurses who can assist with medical and therapy-related activities. We can help with wound and medication management, physical therapy, emotional support, and more.

Specialist Disability Accommodation

Our SDA homes are designed exclusively to accommodate people with extreme functional impairment and high-level care needs. As a registered and trusted NDIS service provider, families can rely on us knowing that their loved ones with complex support needs have comfortable accommodations.

Once we have completed the support needs assessment, we will work with you to develop a personalised service plan. This plan will outline the services and support we will provide and how often these will be delivered. We will also identify any other health professionals or services you may need to be involved in your care.

Start Receiving Support

Once the service plan is finalised, we will provide you with the support and services you need. We will work with you to ensure that these are delivered in a way that meets your individual needs and preferences. We will also review your service plan regularly to ensure that it continues to meet your needs.
